Many Chicago neighborhoods are home to some of the highest incidences of obesity, diabetes, hypertension and heart disease in the city.

We are working to eliminate these health disparities, by designing, implementing, and evaluating community-based initiatives that address the social determinants of health, be they racial, social, economic or environmental, in some of Chicago's most vulnerable neighborhoods.

We are all in this together

Several health disparities exist between people of color and others. Doctors are seeing diseases that you’d expect to see in older patients—heart disease, diabetes, stroke—in relatively young people of color. While genetics may play a role in certain instances, socioeconomic factors have a significant impact on how people of color are able to prevent and treat certain conditions.
